How to become a patient
Call the office
Call (307) 778-3121, Monday to Friday, 8 AM to 5 PM.
Pick your visit
Schedule a new-patient visit, or if you are sick now, ask for a same-day sick visit. You can establish care at that same visit.
Bring the basics
Bring the items listed below to your first appointment, and you are set.
What to bring to your first visit
- A photo ID
- Your insurance card
- A list of all current medications, including doses, vitamins and supplements
- Any allergies to medications
- The name and phone number of your previous doctor
- Vaccination records for children
- Any school, sports or work forms that need to be completed
Transferring your medical records
Your past records help your new provider see the full picture. Ask the front desk for a records release form. Once you sign it, the office can request your records from your previous doctor.
Insurance
Coverage depends on your specific plan and network, so the fastest way to know is to call (307) 778-3121 with your insurance card in hand. The front desk will check your coverage before your first visit so there are no surprises.
