How ongoing care works

  1. A full review. Your history, current medications, recent results and what has or has not worked.
  2. Labs and baselines. Tests such as A1c, a lipid panel, kidney function and thyroid levels, drawn here in the office.
  3. A plan you can follow. Medications, lifestyle steps and targets, explained in plain language.
  4. Regular follow-up. Check-ins at the right interval for your condition, with adjustments as your numbers change.

Why one family practice helps

Many people with one chronic condition have two or three. When one provider manages them together, medications are checked against each other, labs are not repeated by different offices and changes are easier to spot. Your provider also handles vaccines, yearly physicals and sick visits in the same chart.

Labs without a separate trip

Most chronic conditions need regular lab work. At this practice, blood is drawn in the office and processed by LabCorp, and results go straight to the provider managing your care.

Moving your care here

If you are managing a chronic condition and need a new primary care provider, bring your medication list and any recent results to your first visit, and sign a records release so the office can request your history. Read more on the new patient page.

Frequently asked questions

How often will I need to be seen?

It depends on your condition and how well it is controlled. Many patients are seen every few months, and more often when a medication is being adjusted.

Can you refill my current prescriptions?

After you establish care, your provider can review your medications and manage refills. Bring your medication list or bottles to your first visit.

Do I need to see a specialist?

Many chronic conditions are managed well in primary care. When a specialist would help, your provider coordinates the referral.
